Why do some reliability implementations succeed and others fail? Leslie McHattie, Senior Reliability Practitioner at Ivara, has seen it all --some programs are highly successful, most are "satisfactory" and some fail. Trying something new without a plan to manage and sustain the improvement is what we call a silver bullet approach and tends to be very short lived, eventually becoming the flavor of the month.

If the same tools result in variable outcomes, it is evident that the reason this happens does not reside in the tool itself, but rather in how it is managed and applied.
