Organizations must also know how to deal with human error when it is a significant part of the problem, and how to use typologies for identifying human error diagnosis and a cycle for identifying the actual cause.

This event with quality and operations consultant Duke Okes, FASQ, CMQ/OE, will discuss the issues and most challenging questions that organizations face when implementing root cause analysis and corrective action. This boot camp is divided into three comprehensive sessions covering the RCA process and the tools that can aid diagnosis; how to diagnose and deal with human error; and the cognitive and organizational issues that impact effectiveness.
