If everyone in the organization all the way down to the custodian knows that a piece of equipment is troublesome, the solution can lie in simple PM optimization. Reliability-Centered Maintenance is about detective work, finding the failures where the loss of function will not be evident to the operating crew under normal circumstances on their own. Many of these are protective devices that are already in the failed state and you don't know it. This is one barrier found typically in more reactive organizations.
