For many organizations, it is run-to-failure (RTF) although they don't recognize it as such. While some groups have no real proactive maintenance practices, others have invested in and continue on with Preventive and Condition-Based Maintenance programs that are ineffective for a number of reasons. The ineffectiveness takes root in many forms. It could be the lack of true partnerships (Engineering, Operations, or Management) in the organization, equipment availability, the wrong procedures or lack of specification, or the equipment has changed and the PM never updated as examples.
