Reliability engineering not only involves the use of statistical tools (e.g., Weibull analysis, RCM, system simulation, etc.) but also involves the use of efficient management methods. Failure reporting and root cause analysis are such methods and essential elements of a reliability program. These two activities constitute what is known as FRACAS, which compliments existing systems such as historians and Computerized Maintenance Management Systems (CMMS) while providing the foundation of various reliability engineering activities. In this presentation, we will briefly discuss the history and the role of failure reporting, analysis and corrective action system (FRACAS) in a reliability program, as well as its links to other methods such as RCM and reliability analytics.
